Create a Custom Form

As of February 19th, 2021, Product Builder is unavailable for new installations. If you are currently using Product Builder, you will be able to continue using the app as your account will now be grandfathered.

Overview

With Product Builder, you can create a custom form to gather information from your customers.

Custom forms allow you to offer the following option types to your customers:

  • Dropdown menu
  • Scrollable List
  • Single Checkbox
  • Checkboxes
  • Short Text Box
  • Paragraph Text Box
  • Radio Button
  • File Upload
  • Number Field
  • Email Address Field
  • Color Selection
  • Date Picker
  • Telephone Number Field

 


 

Set-Up

  1. From the Shopify admin, select Apps.
  2. Select Product Builder.
  3. Select Edit Steps next to the Product Builder where you would like to add a custom form.

  4. Select Create Custom Form.

  5. Enter a Step Name.

  6. Select Upload Image to add an image for the step. (optional)

  7. Enter a Step Description Title.

  8. Enter a Step Description.

  9. Enter a Field Label Name.

  10. Select an Option Type.

  11. Enter Values, one per line.

  12. Select Required if you want a required value entered here.

  13. Select Add Option.

  14. Repeat Steps 9 - 13 until you have added all options.

  15. Select Create.
